公的年金制度では、被保険者が老齢・障害・死亡のいづれかの状態になったときに年金をはじめとした給付が行われます。
老齢と障害は本人に支給され、死亡したときは遺族に年金が支給されることになります。
障害年金の場合、初診日（障害の原因となった傷病で初めて医師の診察を受けた日）に厚生年金に加入していれば障害厚生年金と障害基礎年金の両方が支給対象となります。一方初診日に厚生年金に加入せず国民年金に加入していた場合または20歳未満だった場合には障害基礎年金のみが支給されることになります。
障害年金は障害の等級によって年金額が異なり、障害等級は１級から３級まであります（３級まであるのは障害厚生年金のみで、障害基礎年金は１級と２級のみ）。
そのため、初診日に厚生年金に加入していて障害等級１級または２級の人は、障害基礎年金と障害厚生年金の２つが支給されます。障害等級が３級の場合は障害厚生年金のみの支給となります。
一方、初診日に国民年金に加入していた人の場合、障害等級１級または２級ですと障害基礎年金のみが支給されますが、障害等級が３級だった場合には障害年金は支給されません。
少し分かり難いかもしれませんが、初診日の時点で自分がどの年金制度に加入していたかによってもらえる障害年金の額が大きく異なりますので、ご注意ください。
